The Registered Nurse (RN) CM is responsible for promoting patient-centered care by coordinating the plan of care for the patient stay, managing the length of stay, ensuring appropriate resource management, and developing a safe appropriate discharge plan in collaboration with the multidisciplinary team. The RN CM facilitates the progression and transition of care using established criteria and in conjunction with the multidisciplinary team. The RN CM will coordinate activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization.
You will be responsible and accountable for coordination of services through an interdisciplinary process that provides a clinical, financial, and psychosocial approach through the continuum of care.
* You will, through concurrent clinical case management, assess patients to determine appropriateness of admission, continued hospitalization, clinical resource consumption, progressive care management, as well as appropriate level of care.
* You will do discharge planning that will begin at the time of (or prior to) admission, and reassessed ongoing throughout the course of hospitalization.
* You will monitor Quality and Risk Management issues and education needs that will also be monitored for and reported to the QRM DEPARTMENT as a part of the CQI process.
Associates Degree or Nursing Diploma. 2+ years' experience in case management OR 3+ years' experience in clinical nursing required.
* Ability to compile, evaluate and report statistics to members of the team and utilize the information to facilitate process improvement activities.
* Located in Aurora, Colorado, The Medical Center of Aurora ( offers over 346 beds. It has five medical campuses, including the Main South Campus, the North Campus Behavioral Health and Wellness Center, Centennial Medical Plaza, Spalding Rehabilitation Hospital, and Spalding Rehabilitation at P/SL. The Main Campus is a Level II Trauma Center with Primary Stroke Certification and Chest Pain Center accreditation, serving the eastern metro area and I-70 corridor. The South Campus has a Level III NICU.
