As a Home Health Licensed Practical Nurse (LPN) for Faith, you will provide one-on-one skilled nursing care to patients using a comprehensive, patient-specific care plan set by the RN Case Manager which establishes goals based on nursing diagnosis and incorporates preventative and rehabilitative nursing actions. You will have the support and collaboration of a full team of healthcare professionals

About the Role:

As a Home Health Licensed Practical Nurse (LPN) for Faith, you will provide one-on-one skilled nursing care to patients using a comprehensive, patient-specific care plan set by the RN Case Manager.


Responsibilities:

  • Providing skilled nursing in the home as ordered on the plan of treatment and nursing plan of care.
  • Assessing patients' condition, communicating, and properly documenting any pertinent information about the patients conditions.
  • Promoting patient/family/caregiver education taking into account the patient's needs and the standards of care.
  • Responsible for performing wound care, disease management, and medication reviews in the home setting as directed by physician and supervising RN.
  • Functioning as a team member by collaborating with the patient, physician and interdisciplinary team.


Qualifications:

LPN QUALIFICATIONS:



  • Have current, valid unencumbered Missouri & Kansas State Board of Nursing LPN license (compact or multi-state license preferred).
  • Have a minimum of 1 year of home health experience.
  • Possess a valid driver’s license, reliable transportation, and auto Insurance.
  • Have or obtain a current CPR certification.


Pay range and compensation package:
