Redeemer Health Home Care and Hospice is seeking an experienced Home Care RN to join our team in providing quality, compassionate care for our patients and their families. This Home Care RN position will primarily provide patient services in Rhawnhurst (19152) and surrounding areas, and is eligible for a $3k sign on bonus. medical insurance eligible the first of the month following start date

Home Care RNs enjoy a flexible schedule, autonomously providing 1:1 direct care in patient homes with the support of an encouraging and experienced clinical team. You will teach patients how to manage their own disease processes in their own environment, focusing on quality care and evidenced based practice.
SUMMARY OF JOB: A Home Care Visiting RN provides safe, holistic care to a population with multiple co-morbidities and advanced chronic illnesses. The RN educates regarding self management and provides support to meet the needs of the patient, family, and community.

1 year of home care experience preferred; able to work weekend, holiday, and on-call hours; able to demonstrate professional competencies in a home care setting; able to efficiently utilize technology (cell phone, computer); able to travel in service area.
