The registered nurse will provide primary and urgent care services to adults, according to certification, at one of PHMC's Medical Respite Programs located in North and West Philadelphia. The Respite Registered Nurse provides triage of acute health problems. coordinates medical care provided by community healthcare providers and functions as a team member with the Mary Howard Health Center and other PHMC Health Center staff. All services are provided in collaboration with the medical respite clinical, case management, and nursing staff. Supports, leads, and directs clinical/operational processes in the Medical Respite
* Supports medical facilities, admissions coordinators and other referral sources in assessing and accepting referrals into the respite.
* Provide medication management using specified guidelines outlined by the medical respite standards.
* Observes ambulatory care guidelines for infection control (handwashing, care of environment, use of safety syringes and sharps containers, use of personal protective equipment).
* Evaluates and treats patients in accordance with clinical best practices for post-acute care in a medical respite care setting.
* In collaboration with the case manager, connects clients to primary care, behavioral health, family violence or other social services as needed.
* Provides client-centered health services that incorporate self-management plans.
* Provide ongoing management, education, and support of patients in all phases of treatment including scheduled nursing visits medication teaching, monitoring, pill counts, observed dosing, and medication refills.
* Exhibit non-judgmental, empathetic and supportive approach when communicating with patients/families and staff.
* Administers medications and manages acute and chronic health problems using evidence-based protocols.
* Provides patient/education, health counseling, and health advocacy services to patients and significant others. Be willing to work evenings and weekends, as deemed necessary.
* Maintains positive relationships with funding sources and community organizations.
* and comply with departmental policies when sharing/documenting relevant patient care data.
* May assist with scheduling of staff and supervision of the Licensed Practical Nurse (LPN), and other residential staff or interns.
* Assists in the oversight of the medical respite workflow.
* Communicates with the Nurse Manager daily to ensure the quality of patient care.
* Assists in the facilitation of external audits/site visits from regulatory agencies and payer sources
* Meets with Nurse Manager and interdisciplinary team weekly to discuss progress of patient care.
Understands and adheres to PHMC compliance standards as they appear in the PHMC Code of Conduct, Whistle Blowers and Conflict of Interest Policies
* Keeps abreast of all pertinent federal, state and PHMC regulations, laws, and policies as they presently exist and as they change or are modified
* Comply with HIPAA and Confidentiality Policies and Procedures as they apply to the job
* Comply with Department of Public Health (DPH), The Joint Commission and other accreditation and regulatory agencies standards
* Adhere to all PHMC Policies and Procedures
* Knowledge and adherence to Infection Control and Environment of Care Guidelines and Procedures as described in the annual education module
Extensive knowledge of current health care needs of diverse populations in an urban environment.
* Knowledge of Electronic Medical Records preferred, but not required.
* Utilize Microsoft Office Suite (Excel, Word, PowerPoint, etc.)
* Ability to collect adequate client data, conduct a physical exam, interpret physical findings and manage or refer patients as conditions or protocols dictate
* Excellent verbal communication and writing skills
* Ability to prioritize and organize workday to meet changing needs of program and patients.
* Thorough knowledge of primary care and urgent care best practices
Experience in primary and/or care, community health preferred. At least three (3) years of nursing experience.
Bachelor's in Nursing required from an accredited program
