Nurse Practitioner - Long-Term Care Location: Defiance, OHSchedule: Monday-Friday, 8:00 AM-5:00 PM (EST)Work Model: Facility-based | 4 days per week (flexible scheduling)Salary Range: $100,000-$110,000Overview A facility-based, interdisciplinary medical team is seeking a Nurse Practitioner to provide high-quality primary and preventive care to patients residing in skilled nursing and long-term care facilities. This role offers the opportunity to make a meaningful impact on the health and well-being of the elderly population while working collaboratively with nursing staff and physicians.Responsibilities Provide primary and preventive care for long-term care and skilled nursing residents Manage residents experiencing acute changes in condition Support advanced care planning and chronic care management Prescribe medications, including controlled substances, in accordance with Ohio law Order, review, and follow up on diagnostic testing and treatments Communicate regularly with nursing facility staff regarding changes in condition and care plans Collaborate closely with primary care physicians and the interdisciplinary care team Required Qualifications Current, unrestricted Nurse Practitioner license in the state of Ohio Active DEA license Clinical background in adult, family, or geriatric care Prior experience in a long-term care or skilled nursing setting Proficiency with electronic medical records (EMR) systems Ability to manage patients with advanced illness and multiple chronic conditions Strong collaboration skills within an interdisciplinary care team Preferred Qualifications Psychiatric experience Additional Details Non-government position Role is newly opened due to expansion and acquisition of a new facility Client-approved compensation structure Interview process includes timely follow-up within 24-48 hours * We will consider for employment all qualified Applicants, including those with criminal histories, in a manner consistent with the requirements of applicable federal, state, and local laws, including the City of Los Angeles' Fair Chance Initiative for Hiring Ordinance (FCIHO), Los Angeles Fair Chance Ordinance for Employers (ULAC), The San Francisco Fair Chance Ordinance (FCO), and the California Fair Chance Act (CFCA). * As a job position within our Allied division, a successful completion of a background check may be required as a condition of employment.
