**Guaranteed Hours:** None
**Experience:** Minimum 1 full year of Adult Home Health or Hospice experience
**License:** Must have license in hand
**Certifications:** CPR, BLS
**Must-Have:**
- Proficiency in HomeCare HomeBase (HCHB) EMR used within the last 6 months
- Proficiency in OASIS for RNs, PTs, and OTs, used within the last 6 months
**Job Description:**
- Provide, coordinate, and direct the provision of home nursing care based on physicians' orders and agency policies.
- Supervise and evaluate Home Health Aides and Licensed Practical/Vocational Nurses.
- Conduct home visits and attend periodic office meetings.
- Manage an average of 5-8 patients per day, equating to roughly 6-8 productivity points.
- Perform various types of care, including Start of Care, Wound Care, and Follow-up Visits.
- Travel up to a 75-mile radius from the home clinic with mileage reimbursed according to the IRS Standard rate.
- Complete documentation within 24 hours of patient appointments.
- Attend weekly meetings and fulfill weekly productivity expectations of 30 points.
- Available to float to nearby clinics if the patient census is low.
- Familiarity with Point Care EMR system.
- Be prepared for on-call rotations as required by location.
- Must dress in scrubs and have reliable transportation with a current driver’s license and liability insurance.
**Additional Notes:**
- Workweek schedule is Monday through Friday, 08:00 am to 05:00 pm.
- There is a possibility of working a Baylor Shift schedule: 4x10 hours from Friday to Monday, depending on the position.
- The territory covers a travel radius of 20-30 miles from the assigned care center.
