*Trinity Trace Community Care Center* is seeking a *Certified Nursing Assistant Transportation Driver* to join our TEAM

*Essential Job Responsibilities*
* The Certified Nursing Assistant Transportation Driver under the direction of the Director of Nursing Services to provide transportation services for facility Residents using the facility van.
* Work with nursing staff to schedule medical appointments.
* Transport residents to and from their medical appointments.
* In coordination with nursing staff ensure that proper medical documents are accurate for transport to medical visits/appointments.
* Maintain safety and comfort of residents during transportation.
* Keep a detailed log of all transports.
* Maintain the routine van mileage log.
* In coordination with facility maintain and schedule routine van maintenance.

*Qualifications*
* Certified Nursing Assistant (CNA).
* Valid Louisiana Driver’s License.
* Ability to communicate effectively with co-workers, residents, families, and physicians.
* Ability to maintain confidentiality and adhere to HIPAA guidelines.
* Sociable, courteous, and sympathetic.
* Public relations Skills.

*Other Qualifications*
* Has not been excluded from participating in Medicare and Medicaid programs.
* Maintains eligibility to participate in Medicare and Medicaid programs throughout employment.
* Capable of providing emergency care as needed for any resident in the facility.
* Participate in pre-employment and annual Tuberculosis screening.

*Physical Demands*
* Standing, sitting, bending, stooping.
* Ability to walk up and down stairs.
* Ability to assist with loading residents on and off van.
