**Job Description:**
- Conduct comprehensive evaluations of patient/resident status and needs.
- Develop treatment plans and establish realistic, measurable short-term and long-term goals in collaboration with the referring provider, patient/resident, family/caregivers, and other team members.
- Provide and assess the effectiveness of therapy treatments according to department policies and procedures.
- Educate patients/residents, family/caregivers, and team members to support the attainment of established goals.
- Deliver care/treatment tailored to the patient/resident’s age, physical ability, and intellectual development.
- Complete accurate, legible, timely, and effective documentation in line with department policies.
- Communicate clearly and concisely with team members, referring providers, insurance/case managers regarding patient/resident status, progress, needs, and discharge planning.
- Delegate appropriate patients/residents treatments to therapy assistant staff and supervise and direct therapy assistants.
- Conduct reevaluations in collaboration with therapy assistants as required, following department policy and procedures.
- Accurately complete daily billing and schedule patients efficiently with coordination from the receptionist.
- Participate in departmental and hospital-wide educational opportunities for continuing professional development and education.
- Provide a departmental in-service if attending a continuing education course reimbursed by the organization.
- Engage in the Rehab Department’s student program as available and appropriate, serving as a clinical instructor, mentor, and role model.
- Participate in one Rehab Department or organizational project, quality improvement initiative, or committee.
- Adhere to all organizational and Rehab Department safety policies and procedures; report incidents, falls, adverse drug reactions, and equipment issues following hospital policies.
- Participate in the development of and adhere to the department mission, values, and ground rules.
- Embrace cultural changes and implementation within the organization.
