Title: Registered Nurse (RN)


Pay Rate: $50-55/hr


Job Description:
We are seeking a skilled and compassionate Registered Nurse to join our team. As an RN, you will administer skilled nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patients/families, and referring agencies, and assume responsibility for the coordination of care. Homecare Homebase (HCHB) experience is required for this role.

Essential Functions:

  1. Assess home health and/or hospice patients in stable and emergent situations to identify physical, psychosocial, and environmental needs.
  2. Develop, implement, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
  3. Obtain data on physical, psychological, social, spiritual, and cultural factors influencing patient/family health status and incorporate it into the POC.
  4. Assure continuity of quality patient care with appropriate documentation.
  5. Provide care utilizing infection control measures.
  6. Educate patients and/or families regarding care, medications, home safety, etc.
  7. Supervise and provide clinical directions to support staff.
  8. Assess and manage pain, utilizing appropriate techniques and educating patients/families.
  9. Arrange referrals to appropriate agencies when necessary.
  10. Monitor assigned cases to ensure compliance with third-party payer requirements.
  11. Perform on-call responsibilities as assigned.
  12. Provide effective communication and serve as a liaison between patients/families and healthcare professionals.
  13. Attend interdisciplinary team meetings and other meetings as necessary.
  14. Perform other related duties as assigned.

Knowledge/Skills/Abilities:

  • Ability to assess and respond to the needs of patients and families.
  • Effective communication skills.
  • Knowledge of medications and correct administration.
  • Competency in various nursing skills including wound care, IV care, and more.
  • Ability to organize tasks, set priorities, and function under stress.
  • Maintains current licensure and certifications.
  • Commitment to professional growth and competency.
  • Ability to work independently and cooperatively as part of a team.

Qualifications:

  • Homecare Homebase Software Experience - Required
  • OASIS Experience - Required
  • Current state license as a Registered Nurse - Required
  • CHPN certification - Preferred
  • Proof of current CPR certification - Required
  • Current driver's license - Required
  • Reliable transportation and insurance - Required
  • 1-2 years experience as a Registered Nurse in a clinical care setting - Required
  • 2 years Home health experience - Required

Education:

  • Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program - Required
  • Bachelors of Science in Nursing - Preferred

Schedule Notes:

  • Shift: M-F until shift is complete
  • On-call info: On-call required if requested
  • Travel Area: West Indianapolis, Brownsburg, Crawfordsville, Avon, Danville, Greencastle, Camby, Plainfield, Mooresville, Martinsville
