Director - Systems Care Coordinator - Care Coordination


The Director - System Care Coordination at Aspirus is a critical leadership position within the healthcare system, responsible for overseeing system-wide care coordination functions. This includes Utilization Review, Social Work, Navigation, and Case Management across all Aspirus entities. The role demands effective management and strategic planning to ensure the seamless integration and efficiency of care coordination services, aligning with Aspirus' health system and health plan goals.


You’ll be working closely with our Vice President, where your insights will directly shape how we integrate and optimize care throughout our facilities. It's a chance to put your mark on our strategic direction and see the impact of your work first-hand. As the Director of System Care Coordination, you’ll also find yourself managing a dynamic team across critical areas like Utilization Review and Case Management will be key. You’ll lead these teams not just to meet but exceed our high standards of care, ensuring we maintain our commitment to operational excellence.

Safety is a priority for us, and it will be for you too. You’ll spearhead initiatives to enhance our safety standards, making sure that both patients and staff are always in good hands. Financial stewardship is also part of the role. You'll manage the budgets and ensure we're financially sound without ever compromising the quality of our services. You’ll also lead quality improvement projects, where the initiatives are essential for improving patient outcomes and streamlining our processes, allowing us to provide better care every day.


Responsibilities:

  • Strategic Leadership: Collaborates with the Vice President to set strategic directions and goals for the care coordination functions. This role plays a key part in executing the strategic plan for the entire health system and health plan, requiring frequent travel to different locations to enhance visibility and coordination.
  • Operational Management: Manages a wide range of care coordination services across hospitals, clinics, post-acute care, and contracted services, ensuring effective service delivery and patient transitions. Supervises all care coordination personnel, including recruitment, retention, and professional development.
  • Patient and Staff Safety: Focuses on creating a safe environment, participating actively in safety events and service recovery to enhance both patient and staff safety.
  • Fiscal Management: Responsible for budget management and cost containment. Works closely with the Finance department to develop and execute departmental budgets and optimize payer incentive programs.
  • Quality Improvement: Leads evidence-based practice initiatives and quality improvement activities, setting quality standards and participating in performance improvement efforts to increase efficiency and scalability of care coordination services.


Requirements:

  • Educational Background: Bachelor’s Degree in Nursing required, with a Master’s Degree in business, healthcare administration, or nursing.
  • Professional Experience: At least five years of experience in care management or utilization review, with five to seven years of leadership experience in care coordination, case management, or utilization review in an integrated healthcare delivery system and/or health plan.
  • Skills: Strong leadership qualities, strategic thinking, effective communication, and the ability to manage interdisciplinary teams. Familiarity with payer mechanisms and clinical utilization management is essential.
