Position Title: Implementation Consultant – (1) Population Needs Assessment/Population Health Management Strategy

Contract Length: EOY 2024

Start date: ASAP

Location: Remote with some travel to in-person meetings needed (5-10% of time)

Overview: This role will support our client's Medi-Cal expansion and compliance with DHCS requirements and policies.? Specifically focused on the CalAIM Population Health Management Strategy and Population Needs Assessment (PNA) in the Population Health Management Group.

Responsibilities may include but are not limited to:

Understand DHCS regulatory requirements and translate them into action

Create and manage workplan, risk/issues, dependencies, status reports, stakeholder communications, service level agreements (SLAs), and other project/program management needs and documents

Serve as SME and program manager to internal and external stakeholders on the Population Needs Assessment work, identify gaps and ongoing needs, and bring feedback back to iterate on approach

Serve as the appointed representative for the PNA work in various counties throughout California which includes attending meetings (with Local Health Departments, community stakeholders, and other Medi-Cal Managed Care Plans) and serving as the primary point of contact in these counties to intake requests from Local Health Departments and track those requests through completion. Provide progress and reports outs for internal audiences. Includes occasional travel for in-person meetings.

Implement statewide processes needed to support the initiative, which may include, but are not limited to: setting up systems to process data and funding requests, tracking and implementing activities that support quality and health equity, etc.

Work closely with various internal departments to implement processes within client, focus area will vary depending on need.

Monitor and ensure adherence to client and DHCS compliance and regulatory oversight

Complete regulatory deliverables required by the Department of Health Care Services

Among the team, the above roles will be spread across individuals.? Team members will support each other and assignments may change based on priorities.?

Qualifications:

Required knowledge of Medi-Cal health plan operations and/or experience working with Local Health Departments and knowledge of the CHA/CHIP process

Strongly desired—knowledge and understanding of HEDIS/MCAS quality measures

Strongly desired—CA resident to attend in-person meetings

Strongly desired – prior client experience, performance improvement

Preferred—knowledge of population health management principles and health equity approaches

Strong communication skills, written and oral

Savvy relationship skills to connect and manage internal (e.g., clinical, non-clinical, contractors) and external audiences (e.g., county relationships, community-based organization) and from frontline staff to executive stakeholders

Experienced in launching new programs/services to the communities

Proactively drive work in an ambiguous environment and gain alignment, flexible, nimble

Experienced in fast-paced environment and growing scale

Strong analytical and critical thinking skills, detail oriented

Strong in Microsoft Office, Have skills with Visio or like process flow application
