Job Description

Minimum Qualifications/Experience:

  • Bachelor’s degree in public health, nursing, social work, or a related field, with two years of relevant experience; OR LPN with 5+ years of healthcare experience.
  • Additional graduate coursework may substitute for experience, and experience can substitute for education (up to four years).
  • Candidates with experience caring for individuals with dementia are encouraged to apply.

Job Overview:

Promote dementia-friendly concepts, provide community memory screenings, educate the public on Alzheimer’s and dementia, encourage early detection, and foster community-clinical linkages.

Key Responsibilities:

  • Build relationships with community partners and healthcare providers to support individuals with ADRD and caregivers.
  • Offer educational materials, memory screenings, and resources.
  • Track program success and barriers to care, ensure HIPAA compliance, and manage financial records.

Competencies:

  • Decision-making, problem-solving, organizing, independence, presentation, written communication, customer focus.

Skills:

  • Active listening, critical thinking, communication, teaching, systems evaluation.
