We are seeking a dedicated and compassionate Registered Nurse (RN) to join our home health team. The RN Home Health Nurse will provide skilled nursing care to patients in their homes, collaborating with interdisciplinary healthcare teams to deliver comprehensive and individualized care. The ideal candidate will possess strong clinical skills, excellent communication abilities, and a passion for delivering high-quality patient-centered care.


Responsibilities:

  1. Conduct initial and ongoing assessments of patients' health status, including physical, psychosocial, and environmental factors.
  2. Develop and implement individualized care plans based on assessment findings, physician orders, and patient goals.
  3. Provide skilled nursing interventions, including wound care, medication management, IV therapy, and patient education.
  4. Perform comprehensive medication reconciliations and ensure accurate administration of prescribed medications.
  5. Monitor patients' vital signs, symptoms, and responses to treatment, and communicate any changes or concerns to the healthcare team.
  6. Educate patients and their caregivers on disease processes, treatment plans, medication management, and self-care techniques.
  7. Coordinate and collaborate with physicians, therapists, social workers, and other healthcare professionals to optimize patient outcomes and continuity of care.
  8. Document all patient care activities, assessments, interventions, and outcomes accurately and timely in electronic medical records.
  9. Ensure compliance with regulatory requirements, agency policies, and standards of nursing practice.
  10. Participate in interdisciplinary team meetings, case conferences, and quality improvement initiatives to enhance patient care delivery and outcomes.
  11. Provide emotional support and counseling to patients and their families, promoting holistic well-being and quality of life.
  12. Maintain professionalism, confidentiality, and ethical standards in all interactions with patients, families, and colleagues.


Qualifications:

  1. Graduation from an accredited school of nursing with a current license to practice as a Registered Nurse (RN) in the state of Georgia.
  2. Minimum of one year of clinical nursing experience, preferably in home health, community health, or a related field.
  3. CPR certification required; additional certifications such as BLS or wound care certification preferred.
  4. Strong clinical assessment and critical thinking skills, with the ability to make independent nursing judgments.
  5. Excellent communication and interpersonal skills, with the ability to establish rapport with diverse patient populations.
  6. Proficiency in using electronic medical records systems and other healthcare technology.
  7. Valid driver's license and reliable transportation required.
  8. Ability to work independently, prioritize tasks, and manage time effectively in a dynamic home care environment.
