Job Description:
We are seeking a compassionate and experienced Rehab Nurse to join our dedicated team. The ideal candidate will have a passion for rehabilitation nursing and a strong commitment to helping patients achieve optimal recovery and independence. This role requires excellent clinical skills, effective communication abilities, and the ability to work collaboratively with interdisciplinary team members.
Responsibilities:
- Provide comprehensive nursing care to patients undergoing rehabilitation therapy for a variety of medical conditions, including but not limited to stroke, spinal cord injury, orthopedic surgery, and traumatic brain injury.
- Conduct patient assessments, develop individualized care plans, and implement appropriate nursing interventions to promote physical, emotional, and cognitive recovery.
- Collaborate with physical therapists, occupational therapists, speech therapists, and other rehabilitation specialists to coordinate patient care and achieve therapeutic goals.
- Administer medications and treatments as prescribed by physicians and in accordance with nursing protocols and standards of care.
- Monitor patient progress, assess for changes in condition, and modify care plans as necessary to optimize rehabilitation outcomes.
- Educate patients and their families about rehabilitation techniques, self-care strategies, and community resources to support ongoing recovery and transition to home or a lower level of care.
- Document patient assessments, interventions, and outcomes accurately and timely in the electronic medical record (EMR) system.
- Adhere to hospital policies, procedures, and regulatory requirements to ensure quality and safety standards are met.
- Participate in interdisciplinary team meetings, case conferences, and quality improvement initiatives to enhance patient care and program effectiveness.
- Provide emotional support and encouragement to patients and their families throughout the rehabilitation process.
Qualifications:
- Current Registered Nurse (RN) license in the state of [Insert State].
- Minimum of two years of nursing experience, preferably in a rehabilitation setting.
- BLS certification required; ACLS certification preferred.
- Strong clinical assessment, critical thinking, and problem-solving skills.
- Excellent communication and interpersonal abilities, with a focus on patient-centered care.
- Ability to work effectively in a multidisciplinary team environment.
- Flexibility to work rotating shifts, weekends, and holidays as needed.
- Experience with electronic medical record (EMR) systems preferred.
