Job Description:

We are seeking a compassionate and experienced Rehab Nurse to join our dedicated team. The ideal candidate will have a passion for rehabilitation nursing and a strong commitment to helping patients achieve optimal recovery and independence. This role requires excellent clinical skills, effective communication abilities, and the ability to work collaboratively with interdisciplinary team members.

Responsibilities:

  1. Provide comprehensive nursing care to patients undergoing rehabilitation therapy for a variety of medical conditions, including but not limited to stroke, spinal cord injury, orthopedic surgery, and traumatic brain injury.
  2. Conduct patient assessments, develop individualized care plans, and implement appropriate nursing interventions to promote physical, emotional, and cognitive recovery.
  3. Collaborate with physical therapists, occupational therapists, speech therapists, and other rehabilitation specialists to coordinate patient care and achieve therapeutic goals.
  4. Administer medications and treatments as prescribed by physicians and in accordance with nursing protocols and standards of care.
  5. Monitor patient progress, assess for changes in condition, and modify care plans as necessary to optimize rehabilitation outcomes.
  6. Educate patients and their families about rehabilitation techniques, self-care strategies, and community resources to support ongoing recovery and transition to home or a lower level of care.
  7. Document patient assessments, interventions, and outcomes accurately and timely in the electronic medical record (EMR) system.
  8. Adhere to hospital policies, procedures, and regulatory requirements to ensure quality and safety standards are met.
  9. Participate in interdisciplinary team meetings, case conferences, and quality improvement initiatives to enhance patient care and program effectiveness.
  10. Provide emotional support and encouragement to patients and their families throughout the rehabilitation process.

Qualifications:

  1. Current Registered Nurse (RN) license in the state of [Insert State].
  2. Minimum of two years of nursing experience, preferably in a rehabilitation setting.
  3. BLS certification required; ACLS certification preferred.
  4. Strong clinical assessment, critical thinking, and problem-solving skills.
  5. Excellent communication and interpersonal abilities, with a focus on patient-centered care.
  6. Ability to work effectively in a multidisciplinary team environment.
  7. Flexibility to work rotating shifts, weekends, and holidays as needed.
  8. Experience with electronic medical record (EMR) systems preferred.
