Job Summary: Healthcare Associates (HCA) is the academic primary care practice at Beth Israel Deaconess Medical Center. The Manager, Population Health, leads HCA's innovative efforts to improve practice performance in population health towards our vision. Using data-driven approaches, the Manager is responsible for providing clear, strategic, and operational direction to enhance the value of care provided to HCA patients through improved quality performance, improved care retention, and reduced total medical expense.

Job Description:

Essential Responsibilities:
Develops, leads, implements, and evaluates population health improvement initiatives in partnership with the Medical Director of Population Health. Owns, analyzes, and reports quality metrics and data. Utilizes available databases to analyze trends regarding practice performance and quality data. Partners with network and hospital leaders to strengthen the quality, collection, and analysis of population health data. Monitors industry trends to identify innovative, creative, and evidence-based solutions to driving population health performance and promote best practice. Collaborates with the Accountable Care Organization (ACO) and network leaders to align and leverage improvement initiatives to drive performance at HCA. Participates in and/or chairs committees, task forces, and meetings related to population health. Maintains timelines and deliverables for population health-related projects. Develops and maintains strong relationships with providers and staff. Integrates clinical programs and resources into practice workflows with intent to optimize care for patients and care team decision support at point of care. Collaborates with practice leadership to coach, educate, and support medical office staff on population health efforts and initiatives. Manages and has ultimate responsibility for the work of the HCA population health team. Partners with the HCA Director of Nursing and Dept of Medicine Administrative Director for Quality to coordinate, monitor and manage HCA chronic and complex nurse/pharmacy care management programs and monitor, manage, analyze and report on metrics related to those programs. Serves as main point of contact for HCA around measuring, monitoring, reporting and analyzing performance metrics related to ACO participation, including Medicaid ACO, and supports the practice in meeting process, documentation and reporting goals required by ACOs, including Medicaid ACO, and/or quality care standards set by external organizations such as NCQA. Has the authority to direct and support employees daily work activities. Has the direct responsibility to undertake the following employment actions: hiring, termination, corrective action and performance reviews. Direct Reports: 4-6 Indirect Reports: None
Required Qualifications: Bachelor's degree required. Master's degree preferred. 5-8 years related work experience required and 1-3 years supervisory/management experience required Ability to manage data in spreadsheets and/or databases, manipulate and analyze the data, develop graphs and oral and written reports based on this data to disseminate to the practice leadership. Advanced skills with Microsoft applications which may include Outlook, Word, Excel, PowerPoint or Access and other web-based applications. May produce complex documents, perform analysis and maintain databases. Preferred Qualifications: Healthcare experience preferred. Oral Communications: Ability to verbally communicate complex concepts in English and address sensitive situations, resolve conflicts, negotiate, motivate and persuade others. Knowledge: Ability to demonstrate broad and comprehensive knowledge of theories, concepts, practices and policies with the ability to use them in complex and/or unprecedented situations across multiple functional areas. Team Work: Ability to lead and direct multiple collaborative teams for large projects or groups both internal and external to the Medical Center and across functional areas.
