Provide home-based care to patients with illnesses, injuries and chronic diseases—often after discharge from a hospital, rehab or nursing facilityWork with a team of physicians and oversee LPNs, CNAs and aides providing patient careExamine patients, do assessments, document progress and report changes to their physicianEducate patients on their plan of care, prescribed medication, therapy, diet and exerciseCoach family members on the patient’s plan of care, medication and home safety Ensure goals are met and coordinate discharge from servicesA few must-haves for Home Health Registered Nurses:
Active Registered Nurse (RN) License in Colorado or Active Licensed Practical Nurse (LPN) with at least one year as a NurseHome health care experience helpful but not requiredNo Case Manager responsibilitiesGraduate of an accredited school of nursing At least 1200 hours of experience as an RN Health care CPR/BLS Certified (demonstration course required)Pass a background check and drug test (we are required to test for THC)No Covid Vaccination RequiredMust be willing to travel to Greely, Front Collins, Loveland and surrounding areaWhy Work for Interim HealthCare?
