Social Worker, Care Coordinator - Justice Involved

Full-Time + Benefits

M-F, 8-5:00

The Justice Involved Care Coordinator works primarily within the jail system. The care coordinator will provide direct mental health services to a population of identified individuals incarcerated within a county jail in the Eastern Iowa Region; also involved with completing pre committal mental health/substance abuse screens. The care coordinator will provide direct mental health and substance abuse assessments, interventions, and consultations to identified individuals and also interact with patients, family, and support systems to reduce barriers and promote progression of care. Providing assessments, counseling, information and referral, advocacy services, and other resource assistance as needed is also a core function of the care coordinator position.

COVID-19 and Flu Vaccination Requirement: It is required to be vaccinated for COVID-19 and Influenza. Exemption requests based on medical or religious reasons may be submitted, but must be approved for active employment.
