Cedar Creek Hospital of Michigan is a beautiful and state of the art 65-bed facility. Cedar Creek provides evidence-based treatment to adolescents and adults struggling with mental health issues. As part of our success and innovation in the field we are seeking qualified applicants for a Lead Clinical Social Worker LMSW position.

The Lead Social Worker, LMSW provides psychiatric case-management and therapeutic services to assigned patients with diverse needs to ensure there is a coordination of care between the hospital, community, treatment, therapeutic services, education and discharge planning. The Lead LMSW also provides clinical support; makes treatment recommendations and actively coordinate with all members of the treatment team. The Lead LMSW also provides licensure required supervision for LLMSW staff.

Inpatient Clinician - Key Job Duties and Responsibilities:

Completes a thorough psychosocial assessment and collaborates with the patient to develop individualized treatment goals. Collaborate and support members of the inpatient treatment team.  Provide clinical supervision for MSW staff.  Initiate discharge planning and arrange aftercare appointments/referrals. Oversee the treatment planning process with person-centered planning and will coordinate all disciplines in treatment interventions, updating treatment plans every seven (7) days or as needed. Conducts group therapy and other groups as assigned. Facilitate individual and/or family sessions as needed. Documents each interaction with patient that is consistent with hospital standards.

Qualifications

Job Requirements:

Current License in the State of Michigan as a LMSW

Master's level preparation from an accredited school.

Experience in inpatient mental health settings required.
