PRIMARY PURPOSE:

The Licensed Clinical Social Worker (LCSW) is a member of the Social Work Department located in the primary care offices to provide care management services to their patients. The LCSW is primarily responsible for face to face comprehensive biopsychosocial assessments, collaboration with patient’s primary care physician to determine diagnosis and treatment through brief therapy and continued follow-up with patients throughout the duration of the therapy process. The nature of this practice will consist of working with adult and geriatric patients who present with emotional disorders and mental illness in a primary care setting. Patients who present to their primary care physicians will be assessed, evaluated, collaboration with patient’s primary care physician to determine diagnosis and treated by the LCSW.

A LCSW is a member of the Social Work Department and works closely with the entire care team to provide optimal services to the patient. The LCSW demonstrates respect, dignity, kindness and empathy in each encounter with all patients, families, visitors and other employees regardless of cultural background. The LCSW will strive to be a highly accessible position that is responsible for creating a positive impression with patients, physicians and other visitors he/she encounters, both in person and on the phone.

EDUCATION:

Minimum: MSW from accredited school by the Council on Social Work Education

LICENSES AND CERTIFICATIONS:

Minimum:

KY - Licensed Clinical Social Worker (LCSW) Class D Driver’s License

YEARS OF EXPERIENCE:

Minimum: 2+ years clinical social work experience in mental health.

REQUIRED SKILLS AND KNOWLEDGE:

Knowledge of the supervisory process. Knowledge of emotional and mental disorders and mental disorders and the DSM. Excellent communication skills and the ability to coordinate care well with an interdisciplinary team. Ability to manage and prioritize multiple tasks. Good Organizational skills Ability to establish/maintain effective working relationships with patients, clinical employees and the public. Working knowledge of Excel, Windows and Outlook and the ability to learn other computer skills as needed. Warm personality with concern for others. Excellent verbal and written communication skills. Excellent interpersonal skills. Positive attitude Self-directed. Demonstrated ability to work successfully in a team-based decision-making culture. Demonstrated ability to work independently resulting in effective outcomes and on-time performance. Experience in planning and coordinating multi-disciplinary communications strategies, strategic initiatives, and events. Must respond and follow through to requests from customers promptly. Must work carefully and precisely with attention to detail. Must utilize resources wisely. Performs duties willingly and with initiative. Shares necessary information so co-workers can do the same. Cooperates with other departments and work groups.
