Memory Care Coordinator

SUMMARY: The Memory Care Coordinator is responsible for the overall oversight of the memory care unit. The Coordinator will ensure all proper programming is occuring on a daily basis.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Complete MDS and other needed assessments
- Manage staff and day to day operations on the memory care unit
- Run care plan meetings for residents on the memory care unit
- Assist with daily programming when needed
- Work with other department leaders as needed to ensure a smooth operation on the memory care unit
- Educate staff on the Memory Care Unit related to Dementia and programing
- Ensure daily programming is taking place
- All other duties as assigned
QUALIFICATIONS:
- LPN/RN Preferred
- Certified Dementia Practitoner preferred
- Long Term Care Experiance preferred
