Overview of the Position:

We are seeking a highly skilled and motivated Clinical Pharmacist to join our team. The clinical pharmacist will work on an interdisciplinary team adding instrumental drug knowledge and expertise to the patient-centered model at HopeHealth. The Clinical Pharmacist will provide direct patient care while also being available to medical providers through drug information requests and referrals. 

Qualifications (Required)

•Doctor of Pharmacy (PharmD) degree from an accredited pharmacy program. 

•Active pharmacist licensure in the state of practice. 

•Strong clinical knowledge and expertise in medication therapy management. 

•Excellent communication skills, both verbal and written, with the ability to effectively communicate complex medical information to diverse audiences. 

•Proven ability to work collaboratively within interdisciplinary healthcare teams. 

•Detail-oriented with strong organizational and time management skills. 

•Ability to adapt to a fast-paced and dynamic clinical environment. 

Qualifications (Preferred)

•Completion of a PGY1 or PGY2 residency program. 

•Completion of a PGY2 Ambulatory Care residency program.

•Board Certification in Pharmacotherapy (BCPS) or other relevant certification. 

•Minimum of 2 years of clinical pharmacy experience, preferably in an ambulatory care or outpatient setting.

Position Responsibilities

•Provide comprehensive clinical pharmacy services to patients and healthcare providers, including but not limited to: 

•Conducting Annual Wellness Visits as co-visits to optimize medication management and adherence. 

•Engaging in Outcomes MTM (Medication Therapy Management) services to ensure safe and effective medication use. 

•Operating Vaccine Clinics to promote immunization and public health. 

•Conducting Hospital Follow-Up Medication Reconciliations to prevent medication errors and improve transitions of care. 

•Performing additional clinical duties as assigned by the supervising pharmacist or management. 

•Oversee suspect conditions assigned for payors as directed, ensuring compliance with regulations and best practices. 

•Collaborate effectively with interdisciplinary healthcare teams to optimize patient outcomes and promote comprehensive care. 

•Provide patient education on medication therapy, disease management, and preventive healthcare measures to enhance patient understanding and adherence. 

•Participate in continuous quality improvement initiatives related to clinical pharmacy services. 

•Maintain accurate and thorough documentation of clinical interventions, patient interactions, and outcomes in accordance with regulatory requirements and organizational standards. 

•Stay updated on the latest developments in clinical pharmacy practice, pharmacotherapy, and healthcare regulations through ongoing education and professional development activities. 

Physical Requirements:

Must be able to lift 30 pounds. Vision and hearing corrected to within normal limits is required. Must have manual dexterity to key in data; utilize computer, grab, grip, hold, tear, cut, sort, and reach.
