Key Responsibilities:
Conduct comprehensive assessments, including OASIS, to establish tailored care plans. Collaborate with a multidisciplinary team to ensure seamless and efficient patient care. Engage in meaningful education and counseling for patients, caregivers, and families. Implement specialized nursing treatments and preventive measure. Document progress and coordinate services, ensuring all team members are informed of patient updates. Supervise LPNs and home health aides, fostering a supportive environment for all staff.
