Position Summary

The Case Manager- Value Based Program (VBP) functions as a core member of a collaborative care team that involves the patient’s primary care provider and when available, other health providers in the primary care clinic. Manages multi-faceted quality and incentive-based programming and reporting efforts. Provides direction to all clinic-based quality assigned associates and serves as a primary partner in strategic community incentive related alliances. The case manager is responsible for coordinating and supporting health care and transitions of care process. He / she is also responsible for coordinating referrals to clinically indicated services outside the primary care clinic (e.g., social services, mental health specialty care, substance abuse treatment). The case manager may also provide evidence‐based treatments or work with other providers when such treatment is indicated.In addition, the manager will manage care plans for patients with chronic diseases needing routine monitoring.

PRIMARY (ESSENTIAL) DUTIES

Support and coordinate health care with patients care team across the care continuum to ensure smooth transitions of care. 
Ensure effective communication and collaboration with SJRMC Discharge Planning, Physician and facility staff in order to foster continuity of care. 
Works with interdisciplinary team in development of discharge plan of care for high-risk patient population. 
Engages primary care physician with transition of care management and assists with discharge plan and oversight post-acute stay
Conduct post discharge contact/visit with each patient, establishing a relationship that fosters smooth transition through the continuum and for at least 90 days after discharge.
Provides patient education about current condition, recommended treatment, medications, and required follow up for patients with chronic diseases.
Maintains appropriate records and documents treatment recommendations and follow up in the patients’ health record to be shared with treating providers.
Participate in caseload consultation with the patient’s primary care and consulting team as needed. Communicate treatment recommendations or changes to care team and facilitate referrals as indicated and ordered.
Collaboratively develop and manage care plans and institute measures to improve overall health of our patient population.
Serves as a Patient Experience advocate in all interactions and activities associated with role enactment.
Ensure providers receive ongoing education related to quality indicators and documentation integrity of such. 
Ensure the maintenance of an appropriate, continuous, and effective quality improvement program for SJMG‘s multi-specialty clinics. 
Establish and maintain effective, collaborative relationships within the clinic(s) and across departments throughout the Medical Group and SJRMC. 
Provide leadership and oversight of the front-line problem solving for provider group in relationship to all clinic quality programming to include, but not limited to: MIPS, BPCI, BCI, and other value-based care planning initiatives rooted in provider activity. 
Promote partnerships with providers by communicating quality activities, provide feedback on results of practice-specific assessments and collaboratively develop improvement plans as needed. 
Act as primary clinic-based liaison with all community-based quality and share of cost partnerships. 
Participate in and support identified process improvement initiatives and projects.
Display a positive attitude, eagerness to learn, initiative, and ability to adapt and change to new roles and duties. 
Performs other duties as assigned.

Qualifications

High School Diploma or GED and one of the following: Certified or Registered Medical Assistant (CCMA) Idaho Licensed Practical Nurse (LPN) Idaho Licensed Social Worker (BSW or MSW) Excellent communication (oral & written) and interpersonal skills Minimum of three (3) years of clinical experience, resulting in excellent clinical skills & judgment
