Program Description

Catholic Community Services’ Foundational Community Support (FCS) Program provides Supportive Housing and Supportive Employment Services under the WA State Medicaid Transformation Project. Supported Housing and Supported Employment Services are for people with complex needs and unique strengths who benefit from hands-on case management and resource connections in order to be successful in finding and maintaining housing and/or employment. This Medicaid benefit is integrated throughout CCS’ diverse social service programming to support housing search and placement activities. Staff work both independently and as part of a strong team across a range of flexible and dynamic service locations. Position Description

This position works with FCS-eligible individuals, focusing on housing and/or job search and placement, resume building, housing stability planning, community resource linkages, skills coaching, job development, care coordination with providers, and landlord-tenant mediation. Work occurs in a variety of settings including housing sites, clinics, employment sites, shelters, and other community locations. This position is encouraged to develop care plans for individuals that address a range of needs that can impact housing or employment goals, such as behavioral or physical health, financial assistance, and community involvement. Work is done collaboratively along with other housing & shelter as well as health care providers, to promote positive outcomes for individuals with complex needs.

What We Offer

As a full-time position (37.5hrs/week and minimum 30hrs/week), employees are eligible for medical, dental & vision insurance coverage, pension and 403(b) plans. CCS also offers optional enrollment in supplemental cancer insurance, hospitalization and critical illness insurance. Employees accrue generous amounts of sick time and paid vacation, in addition to 13 paid holidays annually. Work schedules typically involve a flexible arrangement of in-person and remote locations.

Responsibilities

General Case Management

Complete initial assessment and enrollment into FCS program: review informed consent documents, and conduct an assessment identifying the individual’s strengths, preferences and needs related to housing and/or employment. Provide collaborative person-centered planning to develop an outline of the care plan, including creating short- and long-term goals for housing, employment, education or meaningful activities. Goals will be specific, measureable, reasonable, attainable, and timely. Continuously and at regularly scheduled intervals, review the plan of care with individuals to make revisions and updates when goals are met or when needs change. Coordinate and link clients to services promoting overall health and stability, such as primary and medical care, behavioral health services, crisis services; financial resources, and legal advocacy and training. Coordinate care across providers to ensure collaborative approach to intervention. Encourage social skill building through role modeling effective relationship and communication skills, affirmative and supportive communication strategies, and conflict resolution techniques. Encourage and provide referrals to resources that encourage independence in community living, such as support groups, skills training, or 12-step meetings. Promote, as needed, the development of independence in activities of daily living skills, such as money management, apartment care, checking mail, etc

Pre-Employment and Employment Sustaining Responsibilities

Offer assistance with resume writing, job applications, and preparation for interviews Provide resources and linkages with community resources for appropriate training or educational programs, such as Department of Vocational Rehabilitation, BFET, Goodwill, or others Share employment related resources with individuals and their support teams, which may include contributing to the publication of an employment newsletter distributed throughout the agency. Individualized job development services that support individuals in searching for and securing a job in the community such as:Identifying and negotiating jobs.Building relationships with employers.Customized employment development, job analysis and job carving. Offer benefits education and counseling to provide an individual with the information about what state or federal benefits they may qualify for, and how those benefits may or may not be impacted by an employment income Provide or coordinate transportation to interviews, orientations or other meetings as related to obtaining employment for individuals who need more in-the-field support Assist individuals in exploring career advancement services and professional development opportunities. Assist with enrollment in higher education or credentialing and certificate programs to expand job skills or enhance career development, Negotiate job accommodations or assistive technology needs with the employers on behalf of individuals as needed. Gather and evaluate accurate and objective information about a job to find the best match for an individuals’ skills and needs. Job coaching, including visiting an individual at their work site to provide hands-on coaching or support in their employment Skills training and coaching, especially in terms of money management, budgeting, and financial planning Provide interventions necessary to assist an eligible individual to sustain competitive work in an integrated setting of his or her choice Communicate with an individual’s supervisor or manager, whether in the presence of the client or not (if authorized and appropriate), to mediate and problem-solve any challenges or needs that may have an impact on job stability.

Qualifications

At least one year of demonstrated experience and ability to provide Supported Employment services, or similar service, and meet one of the following criteria:Be a Certified Employment Support Professional (CESP) by the Employment Support Professional Council (ESPCC);Be a Certified Rehabilitation Counselor (CRC) by the Commission of Rehabilitation Counselor Certification (CRCC);Be a Certified Peer Counselor by the State of Washington;Have a Bachelor’s degree or higher in human or social services from an accredited college or university;Have an Associate’s degree in a relevant field and one year experience providing supported employment or similar services. Have four or more years of demonstrated experience providing supported employment or similar services. Knowledge of the social service providers and community resources that are available for individuals. WA State Agency Affiliated Counselor Registration active within 60 days from hire (HIV/AIDS training required). Demonstrable case management experience including problem-solving/issue assessment skills, intervention planning/implementation skills, and/or crisis intervention skills. Demonstrable oral and written communication skills, team-building skills. Ability to uphold and model the mission, values, and insights of Catholic Housing Services/Catholic Community Services. Demonstrates the necessary attitudes, knowledge and skills to deliver culturally competent services and work effectively in multi-cultural situations. Excellent oral and written communication skills. Basic computer skills with ability to maintain up to date and meticulous records. Criminal history background checks are required prior to employment.

Preferred Qualifications

Knowledge of principles, methods, and procedures of services included under supported employment- Individual Placement and Support (IPS), community support services or comparable services that support client ability to obtain and maintain employment.
