Harm Reduction Linkage Specialist

The Harm Reduction Linkage Specialist will assist with the following:
Conducts individual harm reduction sessions with individuals using substances to provide; treatment options, safer injection counseling, wound care, testing, and goal planning related to Harm Reduction Billing services. Available to conduct the above sessions with patients on a walk-in basis. Conducts rapid and confirmatory HIV/Hepatitis C testing and enters affiliated documentation. Links individuals with treatment according to test results, with an emphasis on Hepatitis C due to its prevalence among PWUD. This may include reminder calls about Hepatitis C treatment appointments and check-ins with patients and providers. Provides harm reduction education as they interact with patients in both the SEP and Drop-In Center. Conducts harm reduction groups. Collects and reports data related to program activity for reports required by New York State Department of Health. Attends initial orientation and training, required annual trainings, and other trainings to strengthen skills and improve program outcomes. Collaborates with and supports their direct colleagues and attends meetings with other programs when required. Enters data timely and accurately to promote patient care delivery and participates in ongoing QI/QA activities and training to improve patient experience and increase data quality where needed. Maintains and protects sensitive information in enterprise systems and uses/shares data in compliance with Information Governance and Security Policies. Ability to travel between offices/ clinic locations as necessary. Ability to handle Protected Health Information (PHI) in a manner consistent with the Health Insurance Portability Accountability Act (HIPPA) of 1996. Other duties as requested.Qualified Candidate will have a Associate's degree in Human Services, Public Health or related field with two (2) years of qualifying experience in care coordination, case management with the following population : person with a chronic illness and or/persons with a history of mental illness, homelessness , or chemical dependence . Candidate must be able to communicate clearly and professionally in writing and verbally, demonstrate familiarity with computers and MS Office software (Outlook, Word) and effective time management and organization skills. Sensitivity to HIV/AIDS, addiction and LGBT issues essential.

Required education/experience: Associate's degree s or two (2) years of qualifying experience.
