Job Summary:

JOB SUMMARY

The Advanced Practice Clinician (NP/PA) is service/physician aligned, managing a distinct patient population. The APC may be required to provide consultation with patients and other healthcare professionals too improve care and outcome for patients with complex clinical acute and chronic wounds; will initiate diagnostic tests and procedures relevant to current status of patient and will develop plan of care that integrates assessment, diagnostic strategies and therapeutic interventions and reflect current evidence-based knowledge, and best practice of care for patients with acute and chronic wounds.

KEY RESPONSIBILITIES

Formulates a differential diagnosis based on the history, physical examination, and diagnostic test results.

Uses advanced assessment, knowledge, and skill to make clinical decision for care of the patient relating to their wound.

Uses prescriptive authority, procedures referrals, treatments, and other therapies in accordance with state and federal laws and regulations.

Provides evidence-based pharmacological agents, treatments, supplies (e.g., topical therapies, dressings) and medical equipment according to clinical indicators and results of diagnostic and laboratory tests in accordance with state and federal laws and regulations.

Synthesizes evaluation data from the patient, community, population and/or institution to determine effectiveness of plans and attainment of goals and outcomes for the wound care patients.

Perform advanced surgical sharp debridement into viable tissue (including fascia, bone and tendon)

Apply cellular and/or tissue-based products for advanced wounds.

Defines expected outcomes that integrate evidence-based practice and input from interprofessional members to address cost and clinical effectiveness of care for patients with acute and chronic wounds.

EDUCATION

NP or PA

Board eligible.

EXPERIENCE:
2 years Wound Care patient care experience preferred with working knowledge of:

Patient Evaluation

Wound Debridement

Use of skin substitution 

Use of appropriate dressing

Coding and available for audit reviews.

Chart documentation within by-law requirements.

Terms:

3 years with ability to auto renew.

90-day termination notice required.

Schedule : Full-time

9 clinic sessions (32 hrs./week)

Administrative ½ day (4 hrs./week)
