Team 11 Covers Queens County; Arverne, Astoria, Bayside, Belle Harbor, Bellerose, Breezy Point, Broad Channel, Cambria Heights, College Point, Corona, Douglaston, Elmhurst, Far Rockaway, Floral Park, Flushing, Forest Hills, Fresh Meadows, Glendale, Glen Oaks, Hollis, Howard Beach, Jackson Heights, Jamaica, Kew Gardens, Laurelton, Little Neck, Maspeth, Middle Village, Oakland Gardens, Ozone Park, Queens Village, Rego Park, Richmond Hill, Rockaway Park, Rosedale, Springfield Gardens, St. Albans, Whitestone, Woodhaven, Woodside and the surrounding areas.

Our Home Care PTs:
Review patient charts and medical history Develop a personalized treatment plan Teach therapeutic exercises and stretches to improve patient’s conditions Use hands-on therapy to promote healing Maintain patient records by keeping track of goals and progress while adapting treatment plans accordingly Work closely with a supportive patient care team including physicians, nursing, social services, pastoral care, home health aides and others for your assigned caseload. Provide patient and family education for health promotion as directed by physician’s plan of treatment. Provide care to the most upmost standardMinimum Qualifications:
New York State PT license Minimum of one year of recent hands on experience as a Physical Therapist. Valid New York State Driver’s License and reliable transportationSalary Range
USD - USD /Yr.
This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience.
