The Enterostomal Therapy Coordinator/WOCN is responsible to assess plan make nursing diagnosis implement and evaluate care for clients with wounds or fecal/urinary diversions and clients with bowel or urinary incontinence. This professional will provide continuity of care of the ostomy incontinent and skin care client by improving the competence performance and understanding of the nursing staff through Riverside Home Health services. The WOCN functions as a consultant for clients with complex draining wounds fistulas pressure ulcers incontinence and skin problems related to fecal and urinary incontinence. Serves as a resource person for nursing allied and medical staffs client family and community. In addition this position must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the client served. Responsible for maintaining an ongoing plan for the nursing staff which will include orientation in-service training and continuing education. Duties of the WOCN include:

Partner with leadership to develop a comprehensive virtual health program that includes: Visit guidelines with monitoring for outcomes. Formulary Managemento Skin and wound care programo ROI Metrics Establish a formal written plan of care for referred patients documents specific nursing orders and develop goals specifying expected outcomes of intervention Assess health knowledge and learning abilities of patient and family; assesses the educational needs of the wound care and ostomy patients and families and develops appropriate plans for teaching Utilize all available resources and consults as necessary with members of the health care team in planning and developing individualized teaching plans¿ Function as a knowledgeable resource person to the staff in areas of expertise related to wound ostomy care pressure sore prevention and treatment and management of the incontinent patient After identifying staff learning needs provide the necessary staff education¿ Participate as preceptor in the orientation of new staff as assigned by supervisor Networking with leadership and supply chain management to assure adequate formulary management of resources. Ensuring adequate wound and skin care protocols. Providing hands-on wound care for all difficult and non-healing wounds. Ensuring all regulatory requirements are met.

Riverside Home Care offers home health, usually provided to individuals recovering from an illness, surgery or hospital stay, and short- or long-term in-home private care for assistance with everyday tasks like cooking and cleaning.

Education: Bachelor's Degree required (must have BSN to acquire WOCN)

Experience: Experience in an acute care of home health setting preferred.

Certifications: RN required. WOCN or CWS also required (must obtain within one year). CPR/BLS required. Must also have reliable transportation to make visits to patient homes each day. Required and acceptable driving record required upon hire and to be maintained during employment
