Title: Empowerment Case Manager

Reports to: Program Director

Location: Catholic Charities, Diocese of Venice Inc. - Naples, FL

Classification: Hourly/Non-exempt 

Catholic Charities Diocese of Venice, Inc. provides services to people of all races, backgrounds and beliefs. We offer a variety of housing and social service programs to individuals, families and communities across ten counties in Southwest Florida.

The Empowerment Case Manager position will be responsible for the implementation of the Empowerment and Direct Assistance Program, to include client selection/eligibility, the enrollment process, follow-up, timelines, and pre- and post-measurement assessments and outcome measures.

Job Responsibilities

Meet with community agencies to develop and maintain partnerships. Meet with clients to determine admissibility for the program and provide orientation. Conduct needs assessment and collaborate with clients to write individualized service plans. Work with clients to meet the goals of their individualized plans. Provide ongoing case management and follow-up. Provide referrals to outside community resources when needs cannot be met. Input all required information into Client Track and ensure adequate and accurate documentation. Evaluate success of clients. Advocate on behalf of clients and the organization. Complete and submit accurate and timely reports. Works harmoniously with staff, supervisor, volunteers, community partners and the public. Maintains professional boundaries with staff, volunteers, and clients. Treats staff, volunteers and clients with respect and dignity at all times. Assists Program Director in maintaining positive relationships with funding groups, regulatory and community agencies. Perform other duties as assigned.

Requirements


Bilingual (English/Spanish). Minimum of a two-year college degree in human services or related field. 2 years of successful experience in working in human service field or related field. Experience in establishing and maintaining records, reports and other data. Demonstrates initiative, self-motivation and strong organizational skills Demonstrates warmth, compassion, empathy, honesty and maturity. Ability to respond appropriately and with sensitivity to the cultural differences among our client populations. Must be proficient in basic office software such as (i.e. Word, Excel, and Outlook). Ability to create computer driven reports. Willingness to work within the framework of Catholic social teachings. Work requires sitting, standing and working at computer terminal. Must have a valid driver’s license and driving record that meets our vehicle insurance standards. Travel to partners and other service providers may also be required.
