Job overview

This is an exciting role which will lead the in-house care home service for Ealing Community Partners, providing enhanced care to people in nursing and residential homes in the borough of Ealing. The service has been running as part of Ealing Community Partners since 2019 and is currently part of a sub-contract arrangement. 
The service will be brought into the Trust from May 2024 enabling greater integration of services within planned and unplanned care, alongside the Older People’s Mental Health Services and third sector organisations; Marie Curie and Dementia Concern.

Main duties of the job

Duties:
. To ensure the delivery of all aspects of the Enhanced Primary Care in Care 
Homes Service Specification in Ealing
. To oversee the governance of the service through robust structures and 
oversight of complaints, concerns, serious incidents, mortality reviews
. To work with operational colleagues to ensure the service is delivered within the financial envelope received from the ICB
. To support workforce recruitment and development 
. Integration with other ECP services and Older People’s Services to provide high quality services and to reduce barriers around referrals and enhance 
efficiencies. 
. To work with the care home managers to ensure care plans are robust and 
shared with staff in the homes. 
. To work with third sector partners including Marie Curie and Dementia 
Concern

The post holder is a key member of the Integrated Care Service Line senior 
management team and is pivotal in the provision of effective leadership in ensuring that positive outcomes for patients are achieved. 

The post holder will work collaboratively with the operational managers to support the delivery of safe, effective, caring, responsive and well led services. 

To post holder will take responsibility for the clinical governance and clinical leadership of the service as the Senior Responsible Medical Practitioner, supporting the on-going development and implementation of clinical governance and quality assurance systems within the Service having oversight of and providing guidance on incident reporting/investigation as well as engaging in the implementation of PSIRF (Patient Safety Incident Review Framework).

A key outcome of the incident review process is that lessons are learnt within the Service, and across the Trust and are shared with partner organisations. The post holder will also lead on reviewing and developing care/ clinical pathways, compliance with national standards and NICE guidelines, clinical training and staff education, audit, quality improvement, research and oversight of the risks within the service.


The post holder will play an important role in engaging with North West London ICB regarding the specification for the service. 


The post holder will develop, agree and deliver a strategy for the service that supports the overall strategic objectives of the Service Line and Trust and takes account of best practice evidence and delivers best value

Qualifications

Essential criteria

MBBS or equivalent medical qualification MRCGP

Qualification or higher degree in medical education, clinical research or management Additional clinical qualifications
