Position Summary:

Under the leadership of the Director, Physician Services, the Nurse Practitioner (NP) is an active member of the Physician Services team that delivers medical care consistent with the strategic vision, goals, philosophy and direction of Physician Services and CRHS. The Nurse Practitioner (NP) is responsible for providing care to patients throughout their life cycles (prenatal, pediatric, adolescent, adult, and geriatric) while emphasizing wellness promotion, illness prevention, care of acute minor illnesses and stable chronic illnesses for all life cycles.

Basic Qualifications:

Education:

Requires a Master’s degree in Nursing with specialty in Family or Adult - Gerontology

Experience:

Requires two to four years of work-related experience or any equivalent combination of education, training and experience.

Licensure, Registrations & Certifications: Requires and maintains current state license as a Registered Nurse. Requires and maintains current state certification as an Advanced Practice Nurse Board Certification in one of the following: FNP-BC, AGACNP-BC, or AGPCNP-BC Maintains a current certification in Basic Life Support (If BLS verification is not current upon hire, it must be obtained prior to hire date). Requires and maintains a current valid driver’s license and proof of minimum automobile insurance.

Essential Job Responsibilities:

Responsible for the delivery of care within the long term care facilities as directed by sponsoring physician(s) and within the parameters of the long term care facility authorized protocols. Performs diagnostic procedures, provides medical care and develops and evaluates therapeutic plan of care for assigned residents congruent with authorized protocols. Able to utilize initiative and independent action as required within the framework of authorized professional nursing practice responsibilities. Maintains practice within current legal limitations and guidelines. Must demonstrate knowledge of the principles of growth and development for ages served and possess ability to assess data reflective of the resident’s requirements relative to developmental and cultural needs.  Must demonstrate ongoing competency in basic and special technical skills Examines patients with specific acute complaints or stable chronic illnesses, initiates treatment for identified conditions according to written protocol, and works closely with clinic physicians or primary care in referring patients with significant abnormal findings. Provides appropriate level of clinical and educational services as needed that is age and condition appropriate. Participates in performance improvement (PI) projects. Provides assistance to the physicians on epidemiological issues pertinent to long term care. Maintains chart documentation, tracking and reporting systems as indicated for continuity of care. Follows all Physician Services and CRHS policies and procedures and ensures compliance with all federal and state laws, and all other regulatory requirements. Mentors new employees as requested. Completes all required professional education including continuing nursing education hours and mandatory in-services to maintain professional licensure. Performs other related job duties as assigned. The LTC Facility employed APRN CAN do the following per CMS Regulatory Standards Alternate required SNF visits that must be done by MD (after “Initial”) Any medically necessary visit for Skilled Nursing Facility or Nursing Facility patient. The LTC Facility employed APRN can NEVER do the following per CMS Regulatory Standards: “Initial” Skilled Nursing Facility visit and plan of care that must be done by MD/DO Alternate required SNF visits that must be done by MD/DO Any required visit for Nursing Facility (NF) patients Any Medicare SNF post-hospital care certification or recertification. Will practice within the defined Georgia State Scope of Practice. Adheres to all documentation and regulatory requirements regarding billing and coding of care of patients
