Sierra Vista Regional Medical Center

Job Summary

Ability to work independently with minimal direct supervision. Ability to work cooperatively with health-system and Pharmacy staff. Ability to handle frequent interruptions and adapt to changes in workload and work schedule. Ability to set priorities, make critical decisions, and respond quickly to emergency requests. Ability to exercise sound professional judgment and provide Technician leadership/supervision on a daily basis. Ability to communicate effectively in both oral and written fashion. Responsible for contacting the physician for clarification of orders, including those where the dosage is outside of the recommended range. Ability to meet the pharmaceutical care needs of the patients served in this facility (including: Cardiac, Oncologic, Neonatal (NICU), Obstetrics, ICU-DOU, Ortho and General Medical).

Qualifications

Required Training/Education/Experience/Certification:

Current CA Pharmacist License

Graduated from an accredited School of Pharmacy recognized by the California State Board of Pharmacy.

Has a Bachelor of Science degree in Pharmacy or has graduated from a foreign School of Pharmacy and has demonstrated (through training or direct observation) the ability to perform the requirements listed in the "position summary."

Current California State Board of Pharmacy License.

Desired Training/Education/Experience/Certification:

Has a Doctor of Pharmacy degree preferred

Must demonstrate either through testing or direct observation (preferred) the ability to perform pharmaceutical care needs related to specific age groups (including, but not limited to neonates, pediatrics, adolescent, adult and geriatric.

 Ability to perform "general" computer functions as well as Pharmacy-related computer functions including "order entry," running reports and Pyxis functions.

Pay range: $65.32 Minimum - $104.52 Maximum an hour 

Individual wages are determined based upon a number of factors including, but not limited to, an individual’s qualifications and experience.

If you receive an exemption from the vaccination requirement, you will be required to submit to regular testing in accordance with the law.

This individual’s responsibilities include (but are not limited to): Review and monitor patient's medication therapy and orders written for safety and efficacy considering specific patient conditions and health status; advise patient care team on patient's medication therapy and document such interventions. Order verification and performing medication profile reviews that should include but not limited to checking for duplications, drug- drug and drug-food interactions, allergies, drug appropriateness, dosing, and frequency. Prepare, label, and dispense medications for patients in accordance with physicians' prescriptions; ensure compliance with Federal, State, and local laws and regulations pertaining to the dispensing of drugs and narcotics and the maintenance of required records. Check High Risk IVs, renal dose adjusting medications, IV to PO conversions, initiates/modifies/ discontinues therapy, and mediates black box warning interventions. Ensure that appropriate medications are available for patients and properly procured, prepared, stored, and maintained. Supervise the preparation, maintenance, and dispensing process of drugs and supplies, including technical personnel performing these functions. Maintain relationships with medical and nursing staff; provide timely information pertaining to pharmaceutical supplies, drug usage and compatibility, state and federal regulations regarding drug controls, and Joint Commission standards. Provide for collaborative practice management as per facility-approved protocols for anticoagulation, parenteral nutrition, vancomycin, and aminoglycoside consults, etc. Assist with medication reconciliation and clarification of unsafe and unclear drug therapy orders. Provide education to patients on the use of medications as needed. Participate in the education and training of healthcare professionals (. physicians, nurses, pharmacy technicians, and students) in relation to medication use. Demonstrate regulatory compliance as evidenced by timely completion of nursing unit inspections, as needed. Maintain professional competence with pharmacy department and state licensing requirements via staff development, independent study, and/or professional activities.

Required: Pharm.
D. or BS in Pharmacy

Experience:

Preferred: Hospital experience

Certifications:

Required: Pharmacy license; BLS within 30 days of hire (ACLS and/or PALS if code attendance required);
Preferred: Board Certification in Pharmacotherapy (BCPS), CGP, etc.

Physical Demands:

Lift/position up to 50 lbs. Push/pull up to 50 lbs. of force. Frequent standing and walking. Frequent reaching, stooping, and bending. Manual dexterity, mobility, touch, auditory to perform all the related duties of the position
