We are looking for a Part-Time Case Manager to join our team in Wilmington, Delaware. The Case Manager has primary responsibility for providing quality, sufficient and appropriate case management to clients served in the Impact Life program. The Case Manager meets with clients and their families, connecting clients with professional services and maintaining consistent communication with clients to assess their wellbeing and make changes to their treatment plan as needed. The Case Manager is also responsible for modeling and maintaining high professional conduct and standards.

Please note: this part-time position for **up to 15-hours per week**.

Requirements

General Accountabilities

  • Meet with assigned clients as required to formulate and carry out therapeutic goals.
  • Refer and link clients to appropriate services.
  • Coordinate services with outside providers when needed.
  • Provide appropriate treatment and care to clients.
  • Maintain appropriate written records and Alleva records.
  • Refer clients to appropriate clinical and/or medical staff as needed.
  • Follow-up on all referrals for Impact Life clients and record actions.
  • Participate in recovery planning meetings.
  • Conducts individualized case management needs assessment, prioritizing the data collection based on the client’s immediate condition or needs within timeframe specified by policies, procedures.
  • Conducts ongoing assessments as determined by the client’s condition and/or the client facility’s policies, procedures or protocols and reprioritizes care accordingly.
  • Facilitates groups as required.
  • Works directly with the treatment team to assist in developing an individualized recovery plan which demonstrates collaboration with treatment team.
  • Performs therapeutic interventions as established by individualized plan of care for the client.
  • Provides individualized client/family education customized to the client and their family.
  • Documents all interaction with client including any assessment findings, physical/psychosocial responses to interventions and progress towards problem resolution.
  • Maintains confidentiality in matters related to client, and family.
  • Provides crisis intervention and therapeutic de-escalation.
  • Provides care in a non-judgmental, non-discriminatory manner that is sensitive to the client’s diversity, preserving their autonomy, dignity and rights.
  • Reports any change in condition to appropriate personnel.
  • Works closely with members of the therapeutic treatment team, external agencies, clients and families, and members of other departments.
  • Observes urine testing when necessary.

Additional Preferred Qualification

It is preferred that this person has personal, lived experience in their own substance use disorder and/or mental health disorder recovery. By offering insight into the recovery process based on their own experience, this peer will be able to provide a unique perspective while providing recovery support services. The Case Manager will act as a role model, mentor, advocate, and motivator through the housing admissions and discharge process. Responsibilities specific to the peer recovery support specialist skillset are:

  • Assists in the admission and discharge process by supporting and assisting clients in gaining entitlements.
  • Assists potential, current, and former residents in accessing resources within the community such as food vouchers, housing, linkage to employment services, childcare, and social services.
  • Provides guidance and assists clients in overcoming challenges in their recovery journey.
  • Facilitates psychoeducation groups.
  • Implements recovery/care plans and connects individuals to support services.
  • Educates and empowers individuals by assisting them in developing a strong, stable recovery support system.
  • Assisting individuals with self-care and self-awareness, encouragement and support, teaching coping skills, and promoting the positive benefits of long-term recovery.
  • Responsible for providing quality, sufficient, and appropriate services while maintaining high professional standards.

Job Qualifications

  • High School Diploma or GED Required.
  • Bachelor's degree in behavioral health or similar field preferred.
  • Minimum of two-year experience in behavioral health or similar field.

Skills

  • Ability to work efficiently in a fast-paced environment and reprioritizes work as necessary.
  • Excellent verbal and written communication skills and telephone techniques required.
  • Effective working both independently and as a team member.
  • Ability to work collaboratively with all levels of staff, community members, and key stakeholders.
  • Working knowledge of community resources.

Travel/Special Requirements

  • Requires local travel.
  • Must possess and maintain a car and a valid driver's license
