  • Schedule:  Full time, 40 hrs/week, day shift, varied hours Monday and Wednesday 9 am - 6 pm, Tuesday and Thursday 7:30 am - 4:30 pm, Friday 7 am - 1 pm.

Responsibilities

Serve as a patient navigator to accommodate patients throughout the continuum of care.

  • Coordinate and lead presentations to prospective patients which may include community seminars, expositions and support groups.
  • Provide support with navigating the healthcare system and assist patients with utilizing available community support services.
  • Address barriers to care. Work with Navigation Team as needed and serve on hospital committees as assigned.
  • Work with physicians and the health care team to accommodate patient issues and to ensure quality patient care.
Requirements

Licensure / Certification / Registration:

  • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
  • Licensed Practical/Vocational Nurse credentialed from the Kansas Board of Nursing required.

 

Education:

  • High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.
    • Note: Required professional licensure/certification can be used in lieu of education or experience, if applicable.
