Job Details Overview: As a Registered Nurse with us, you'll play a vital role in planning, organizing, and delivering home care services. We're seeking experienced nurses with a passion for community health education and engagement. You'll utilize community resources to tailor services that meet the unique needs of individuals and families within their homes and communities.
Responsibilities:
- Conduct comprehensive assessments of patients and families at regular intervals to determine home care needs.
- Provide thorough physical assessments and medical histories.
- Continuously reassess patient needs and adjust care plans accordingly.
- Collaborate with physicians to develop personalized care plans based on nursing diagnoses.
- Involve patients and their families in the planning process.
- Implement preventive and rehabilitative nursing procedures.
- Administer medications and treatments as prescribed.
- Offer counseling and health education to patients and families.
- Identify discharge planning needs and implement plans prior to discharge.
- Perform peripheral short intravenous line establishment and administer IV medications under physician orders and with demonstrated competency.
Qualifications:
- Current licensure in the State of New York.
- CPR certification and a valid driver's license.
- Graduation from an accredited school of nursing.
- Preferably, a minimum of one year of recent acute or long-term care experience, ideally in an institutional setting.
- Bachelor's degree preferred, with at least one year of experience in home care.
- While management experience is not required, you will be responsible for supervising Home Health Aides and Personal Care Aides.
- Strong observation, verbal, and written communication skills.
- Excellent problem-solving abilities and basic math skills.
- Nursing skills per competency checklist.
Job type: Fee for Service, Part time, Per diem, Full time
Pay: $75-$105 per visit
