As SCL (Supports for Community Living) Coordinator, you will participate in Plan of Care (POC) meetings and assessments, with SCL/MPW team members or outside agency support staff, as appropriate. The SCL Coordinator encourages implementation of member goals with the objective of increasing self-reliance and independence and ensures that required service documentation reflects progress and concerns. The SCL coordinator ensures that activities are designed based on the needs and interests of members and not only entertain, but they also encourage interaction, relaxation, promote health, and contribute to the overall quality of our members’ days.

Responsibilities

SCL Coordinator responsibilities include but are not limited to:

Participating in member Plan of Care (POC) meetings and assessments, with SCL/MPW team members or outside agency support staff, as appropriate. Collaborate effectively with member case managers and other support providers/agencies to ensure that services are person centered. Encourage implementation of member goals with the objective of increasing self-reliance and independence for members, while also ensuring required service documentation reflects progress and/or concerns. Ensuring member activities are designed based on the needs and interests of members and that they not only entertain our members but also encourage interaction, relaxation, promote health, and contribute to the overall quality of our members’ days Training staff with Behavior Specialist as well as train staff on each SCL member’s individual need while ensuring all SCL goals are met for members on the SCL waiver Ensure all paperwork associated with members covered by the SCL waiver is complete and on time. Ensure compliance with the Federal Preadmission Screening and Resident Review process.

Qualifications

Qualifications include:

High school diploma or GED required, Bachelor of Science Degree preferred; additional requirements as mandated by state-specific regulatory guidelines. Minimum of two years (or full-time equivalent) activities experience preferably in a healthcare setting; additional requirements as mandated by state-specific regulatory guidelines (. Certified Activity Professional). Prior experience working with an SCL provider and/or successful completion of classes under the College of Direct Support strongly preferred. Strong communication and engaging interpersonal skills Working knowledge of Microsoft Office Word, Excel and Outlook. Successful results of background check, and where applicable, additional state-specific regulatory requirements.
