Job Title: Social Worker (MSW)

Job Summary

Acts as an advocate to the residents and families of the MJH long term care nursing home area by delivering a broad spectrum of social work services in a qualitative, empathetic, and caring manner.

Essential Job Functions

·Supports, counsels, and acts as an advocate for residents and family.

·Completes all necessary and required social work documentation to include psychosocial assessments, treatment plans, quarterly evaluations, MDS, supplemental notes, discharge plans etc

·Coordinate and facilitate patients discharges by ordering all services needed post discharge from facility

·Interfaces with interdisciplinary staff members to share information on the resident

·Assists with discharge planning in a rehab setting

·Facilitates support groups

·Attends meetings as needed

Job Requirements

Education

·Master’s degree in clinical social work required

Experience

·Three years of related experience

Licenses/Certifications

·N/A

Abilities Required

·Excellent assessment and counseling skills

·Ability to work in a fast-paced social work position that involves discharge planning from a rehab setting

·Must be computer literate

·Excellent communication and interpersonal skills

·Knowledge of geriatrics/gerontology preferred

·Ability to work closely and well with other disciplines

·Thorough knowledge of psychosocial dynamics of individual & family

Functional Demands

·Normal working condition with adequate lighting and ventilation

Infectious Material Exposure:

·No routine exposure to blood/body fluids/tissues but may occasionally require performing unplanned activities which may involve exposure to blood/body fluids/tissues
