Job Summary

: Reports to physical therapist when re-evaluations are needed and or changes in patient status. Delegates tasks and duties as indicated. Performs documentation per policy and procedures ( progress notes, discharge summary, and multi-disciplinary education and treatment plan forms).

Qualifications

Required:

PTA Licensure in practicing State Associates as Physical Therapy Assistant

Preferred:

Physical Therapy Assistant or Rehab experience
