APN Healthcare Solutions is urgently hiring multiple Inpatient Pharmacists for our client in Solvang, CA for a 13 week contract with a high possibility of extension.

Position Title: Inpatient Pharmacist
Pay Rate: up to $95/hr
Work Location: Solvang, CA
Shift : Days
Hours/day: 8hrs
Contract Duration:13 weeks
License Requirement: RPh, Current licensure in the State of California as a Pharmacist
Certification Requirement: BLS

Job Title: Inpatient Pharmacist

Job Summary: The Inpatient Pharmacist is responsible for ensuring the safe and effective use of medications for hospitalized patients. This role involves working closely with healthcare providers to optimize drug therapy, manage medication regimens, and provide pharmaceutical care in a hospital setting.

Key Responsibilities:

• Medication Management:
• Review and verify medication orders for accuracy, appropriateness, and potential drug interactions.
• Prepare and dispense medications, including intravenous solutions and specialized drug preparations, according to established protocols.
• Monitor patient responses to medications and adjust therapy as needed in collaboration with the healthcare team.
• Conduct medication therapy management and perform medication reconciliation upon patient admission and discharge.
• Clinical Consultation:
• Provide drug information and recommendations to physicians, nurses, and other healthcare professionals.
• Participate in patient rounds and contribute to the development of individualized treatment plans.
• Educate patients and their families about their medications, including proper usage, potential side effects, and interactions.
• Quality and Safety:
• Ensure compliance with hospital policies, regulatory requirements, and standards of practice.
• Participate in the development and implementation of medication-related protocols and guidelines.
• Conduct medication audits and contribute to quality improvement initiatives to enhance patient safety and care.
• Pharmacy Operations:
• Oversee the inventory and management of medications, including ordering, storage, and documentation.
• Supervise pharmacy technicians and support staff, providing guidance and training as needed.
• Maintain accurate records of medication usage and ensure proper documentation for regulatory and accreditation purposes.
