An Access and Adherence Navigator contributes to AHF’s mission by conducting proactive outreach to patients who are enrolled in the AIDS Drug Assistance Program (ADAP) into comprehensive healthcare coverage and assists clients with achieving and maintaining viral suppression through educational materials and linkage to appropriate county resources. Our benefits department works together to ensure our patients have a source of funding for their healthcare and medications. We issue coverage based on funding available from federal, state, and county grants and contracts. We provide the appropriate screenings to ensure proper eligibility based on our contractual requirements. The private insurance industry can be quite challenging to understand. Helping our patients navigate through the system is a vital part of the program. In addition to enrolling our patients into medical funding, we also ensure that our patients remain adherent to their prescribed medication regime. Viral load suppression is the ultimate goal and to aid this goal, our Access and Adherence Navigators screen each patient to help identify any medical or social barriers that could prevent this. Collaboration and networking with medical and social resources both internally and externally is key to the success of this program
