Under the direct supervision of the Nurse Manager/Nurse Supervisor and/or the Director of Nursing, the Registered Nurse is responsible for providing direct patient care to a diverse patient population across the continuum of care.

This position is a full time, 40 hours per week, fully benefited, limited term position. The term can be up to 6 months, with possible extension, dependent on needs.

This role is on-site at our Arden location, Sacramento, CA (95825).

Location Hours:

  • Monday, Wednesday, Friday, 8am-5pm
  • Tuesday/Thursday, 10am-7pm
  • Saturday/Sunday, CLOSED

The specific schedule will be determined at the time of offer and may include rotating Saturdays at our Midtown clinic.

Essential Functions

  • Application of the Nursing process to assess, plan and carry out interventions in the management of patient's care in collaboration with the multidisciplinary care team including medical providers, mental health/behavioral health providers, case management, dental, optometry, registered dieticians, clinical pharmacists, health educators etc.
  • Detailed, systematic documentation of all patient interactions in the electronic medical record.
  • In-person and telephone triage and advice supported by recognized industry protocols; approved by the Chief Medical Officer and compliant with clinic policy.
  • Nursing support for diverse population ranging from newborn to geriatric using culturally competent care.
  • Coordination of post-hospital follow-up appointments for primary care patients.
  • Nurse-led hypertension management through comprehensive One Community Health program.
  • Results review, including lab and imaging results for issues ranging from chronic diseases to sexual health.
  • Patient education and counseling for chronic disease management, medication adherence, preventive care, and wellness promotion.
  • Medication administration as ordered by qualified providers; including PO, inhaled, IM, IV, and intradermal medications.
  • Procedures and patient care as ordered, including venipuncture, wound care, port flushes, etc.
  • Active support of health team members for the continuity of care provided to all patients at One Community Health.
  • Patient scheduling of follow-ups and specialty appointments.
  • Coordinating and follow-up with patient calls and EHR messaging within timelines set by clinic protocols.
  • Leadership to clinical support staff including MAs and LVNs in a team-based model of care.

Additional Duties

  • Convey critical assessment data and observations in a professional manner.
  • Delegation of tasks as appropriate within applicable scope of practice to LVN or MA. Oversight of delegated tasks to ensure complete, timely and appropriate execution.
  • Document all pertinent professional interactions.
  • Complete all required documentation comprehensively and in a timely manner.
  • Identify conflicts and seek consultation as needed for professional resolution.
  • Interact in a collaborative atmosphere with providers, clinic staff and laboratory staff.
  • Participate in quality improvement activities including annual training in quality improvement.
  • This position functions as a member of the One Community Health team and as such actively collaborates with other clinical and non-clinical staff to ensure care is proactive and coordinated. This collaboration may include attendance at daily team huddles, care conferences and trainings. There will be ongoing training as appropriate.
  • This position is responsible for reporting suspicion of child, adult (including spouses), elder, and dependent adult abuse as outlined in Penal Code, section 11166, Section 15630, and Section 1160.
  • Other Duties as assigned.

EDUCATION AND/OR EXPERIENCE AND OR LICENSE/CERTIFICATION

  • Associates in Nursing required, Bachelor's of Nursing preferred.
  • 1 year clinical experience required, 1 year acute care experience preferred.
  • Current active license to practice as an RN in the state of California.
  • Active CPR, BLS Certification.
