Provide home-based care under the supervision of a RN to patients with illnesses, injuries and chronic diseases—often after discharge from a hospital, rehab or nursing facilityWork with a team of physicians, RN’s, and CNAs to deliver quality patient careExamine patients, complete assessments, document progress and report changes to their physicianEducate patients on their plan of care, prescribed medication, therapy, diet and exerciseCoach family members on the patient’s plan of care, medication and home safety Collaborate with interdisciplinary team to coordinate patient care and ensure goals are met and plan for discharge from servicesA few must-haves for Home Health LPN's:
Graduate of a practical (vocational) nursing program for LPN and has a current GA LPN licenseMinimum of 1 year (LPN) experience in home healthcareHCHB charting experience highly preferredCPR certification, negative ppd screening or chest x-rayDrivers license, reliable transportation & auto insurance Knowledge of state and federal home health regulationsClinical proficiency in nursing specialty, strong communication skills and compassionAble to pass federal and state required criminal / abuse background checks and drug screenWhy Work for Interim HealthCare?
