Position Highlights & Job Responsibilities:
Physician-led, patient centered team-based environmentProviding full spectrum Family Medicine & Adult Primary Care, focusing on value-based care, and supported by a robust team of specialistsFull-time; Work-Life Balance; Flexible Scheduling Potential for limited inpatient/hospital rounding as part of full-time, 40-hour per week scheduleTeamwork; Evidence-based MedicineAutonomy to practice at the peak of your licenseSupported to grow your practice and patient panelMust be comfortable working with Advanced Practice CliniciansPrimary Responsibilities:
Direct and manage a complete continuum of high-quality care within the care teamTreat chronic diseases and disordersReviews patient file/record, including allergies, problems, medications, and immunization statusElicits and records information about patient’s medical historyOrders or executes various tests, analyses, and diagnostic images to provide information on patient’s conditionAnalyzes reports and findings of tests and examination and diagnoses condition of patientWhat makes an Optum organization different?

Unrestricted licensure in the state of New York or has New York application in processActive and unrestricted DEA License or ability to obtain prior to startResidency trained and Board Certification or Board Eligibility in Family Medicine and/or Internal Medicine
New York Residents Only: The salary range for this role is $, to $, per year. Salary Range is defined as total cash compensation at target. The actual range and pay mix of base and bonus is variable based upon experience and metric achievement.
