Job Description

Purpose of Position:

Under the direct supervision of a Registered Professional Nurse or other licensed nurse, the Nurse Aide is responsible for providing routine daily nursing care to assigned patients, ensuring their safety and wel- being in accordance with all applicable laws and regulations.

Responsibilities:

1) Obtains and records vital signs (temperature, pulse, respirations, blood pressure, pulse oximeter), weight, height, intake and output and informs the Registered Nurse of findings.

2) Prepares patient for examination and/or special procedures and remains with patient during procedure if needed.

)3 Observes patient's condition, mental state and social behavior and report observations to the Registered Nurse.

4) Prepares patient for meals and assists with feeding if necessary. Distributes nutritional supplements to patients and records on Intake &Output sheet.

5) Assists and accompanies patient in admitting, discharges and transfer processes, assuring comfort and safety.

6) Assist patient in activities of daily living.

7) Transports and accompanies patient to and from unit when necessary.

8) Keeps unit, utility rooms, treatment rooms and storage rooms in a neat and orderly condition. Keeps dressing cart and clean utility room well stocked with supplies at all times. Checks supplies for expiration dates, including surgical supplies.

9 Performs AM and PM care.

10) Participates in pain management by informing the Registered Nurse of patient's complaint of pain. 11) Monitors patient on suicidal precautions, documents per hospital policy and notifies the Registered Nurse of patient's behavior.

12) Monitors patient on fall precautions and completes the flowsheet per hospital policy.

13) Collects routine specimens (urine, stool, sputum).

14) Performs post-mortem care.

15) Participates in Nursing and interdisciplinary team conferences to assess patient needs and plan nursing care.

Minimum Qualifications

1. Graduation from elementary school. 
2. Successful completion of a basic skills training program for Nurse Aides, or its equivale
