Provide home-based care to patients with a prognosis of six months or less to live, focused on quality of lifeWork as part of an interdisciplinary team comprised of a physician, RN, CNA, HHA, social worker, chaplain and volunteers to carry out the patient’s plan of carePerform nursing services which are planned, delegated, and supervised by the registered nurseAssess and provide patient and family/caregiver education and information pertinent to diagnosis and plan of careTake a holistic approach to care that engages the patient’s mind, body, spirit and familyA few must-haves for Home Health Licensed Practical Nurses:
Graduate of an accredited nursing program and active Licensed Vocational Nurse (LVN) license in CaliforniaMinimum of 1 year of nursing experience, ideally in hospice or home healthcareCPR certificationKnowledge of state and federal home health regulationsClinical proficiency in nursing specialty, strong communication skills and compassionWhy Work for Interim HealthCare?
