Avante at Ocala is seeking a Social Services Director to ensure that the medically related psycho-social needs of our residents are met and maintained on an individual basis. We are a 133 bed Skilled Nursing Facility who offers short term post acute rehab and long term care. Complete assessments, discharge planning, resident customer service program, documentation in medical record, IDT team meetings, ancillary services for residents, and work closely with our medical providers. Prefer Masters Degree but will hire with a Bachelors Degree not limited to Sociology, Psychology, or Rehabilitation CounselingMinimum of 1 years experience in Long Term Care.At least 1 year of supervised Social Work experience in a Health Care setting working directly with individuals.
