Responsibilities

  • Implementation ofphysicians prescribed plan of care
  • Patientassessments and coordination ofcare
  • Administration of prescribed medicationtreatments and therapies
  • Compliant timelydocumentation of care provided in system ofrecord
  • Health promotion teaching and trainingof family members
  • Responsible for accepting andwriting verbal orders from physicians as required and for obtainingcosignature of Case Manager on all verbalorders
  • Follow up with physician client familymembers staff and Case Manager on client problems concerns and/orchanges in client condition
  • Performs otherappropriate duties asassigned

Qualifications

  • Must have and maintain an activeunencumbered license (RN/LPN/LVN) in the state in which theclinician will practice
  • Current CPRcertification from American Red Cross American Heart Association orBLS
  • TB skin test (current within last 12months)
  • Six months prior handson nursingexperience preferred but not required
  • Musthave reliable transportation

PositionOverview
The Licensed Practical Nurse (LPN/LVN) isresponsible for providing and documenting skilled nursing careunder the supervision of a Registered Nurse in accordance with thedeveloped care plan and physicians orders for each individualpatient while adhering to confidentiality standards andprofessional boundaries at all times.

EssentialJob Functions
Responsible for the delivery andcoordination of quality patient care in compliance with physicianorders.
Continuously observes and assesses patientcondition and care needs and reports changes in condition to thesupervisor and/or physician as appropriate.
Documents allactivities assessments nursing actions responses and coordinationof care in a timely manner whenever care is delivered.
Participate implement and update the nursing care plan.
Takes appropriate nursing action based on assessment and achievesexpected outcomes.
Recognizes changes in patient needsand responses requiring intervention and implements care to preventrisk or reduce risk.
Accepts responsibility for personaland professional accountability by complying with Aveanna policiesstate and federal regulations accrediting bodies and the NursePractice Act.
Provide care utilizing infection controlmeasures that protect both the staff and the patient according toOSHA standards.
Educates the patient and family regardingthe disease process selfcare techniques and prevention strategiesand in meeting the patients nursing needs.
Maintainknowledge of competencies related to the nursing profession byparticipating in educational programs continued education unitsinternal learning management skills and skillevaluations.

Requirements
Graduate ofan accredited school of nursing.
Current unrestrictedstate license as a Licensed Nurse in the state ofpractice
Current CPR certification
Demonstratedproficiency in clinical assessments documentation and compliancewith nursing care and policies and procedures
Additionalstate specific requirements:
South Carolina One (1) yearof pediatrics experience
California One (1) year ofexperience required working under current nursing license
Louisiana One (1) year of experience required working as a licensednurse
Continuing Education as required bystate

Preferences
Six (6) months ofrecent experience as a Licensed Nurse in a clinical caresetting
Home health experience
