Overview
Helping Hands Pediatrics is located in South Bloomfield, OH. This is a full-time, benefits-eligible position working Monday-Friday 8:00 a.m. to 5:00 p.m. The Community based RN emphasizes health promotion and disease prevention as they assist patients and families to move rapidly, uneventfully, safely, and as effectively as possible through their episode of care. The Community based Ambulatory RN delegates, supervises and provides patient care; plans for education and discharge; and provides for patient safety and quality of care.Demonstrates competencies defined (*) in Community based AMBULATORY RN responsibilities for providing age-appropriate care to patients from the age of birth through the lifespan, and select adult patients.In addition, the Community based Ambulatory RN assists the Clinical Leader/Program Manager/Program Supervisor in the supervision of clinical operations, planning, evaluating and controlling program services and ensuring that internal and external standards are met.

Responsibilities
*1. Expertly uses and serves as a resource for the nursing process, Standards of Care, clinical pathways, and clinic specific competencies, to:a. Complete the appropriate health status assessmentb. Document a plan of care on the medical summary and/or clinic visit sheet or electronic medical record as appropriatec. Delegate patient care as appropriated. Evaluate patient care and patient’s response/progresse. Reassess patients and revise plan of care as appropriatef. Supervise patient careg. Plan dischargeh. Plan, provide, delegate, and evaluate patient/family education as appropriatei. Collaborate effectively with all health care disciplines to facilitate optimal patient and family health and full potential*2. Demonstrates awareness of and support for the principle of Family Centered Care.*3. Communicates patient care needs/expectations/priorities to assigned Licensed Practical Nurses (LPNs), Patient Care Assistants (PCAs), Clinical Assistants (CA) and allied health personnel.*4. Effectively relates and interacts with patient, families, and health care team and serves as a role model for others. 5. Supervises clinic functioning as appropriate.*6. Effectively facilitates patient care through one or more of the following: a. Delegating and coordinating care for patients b. Care conferences c. Clinical rounds d. Lead nurse responsibilities e. Other coordination activities 7. Provides nursing care focused on positive patient and family outcomesa. Appropriately implements Licensed Professional Initiated Protocolsb. Identify patients and families at increased risk c. Collaborate with internal and community resources to promote a healthy lifestyle and prevent escalation of health conditionsd. Health supervision of patient and families to facilitate access, immunizations and screenings foro Safety and injury preventiono Healthy weight and nutritiono Smoking cessationo Developmental screeningso Oral healtho Behavioral health 8. Maintains professional standards and serves as a role model by setting professional goals and participating in activities to meet goals. 9. Facilitates clinic operations within clinic budgetary standards: a. Assists with daily staffing as appropriate b. Review data to justify and/or correct variances as appropriate c. Participates in clinic budget preparation as appropriate d. Participates in review and revising of visit charge sheets as appropriate10. Provides leadership by delegating and supervising clinic maintenance activities: a. Completion of daily visit charges b. Infection control activities c. CLIA maintenance as appropriate11. Participates, delegates and enlists staff participation in Patient Care Services and Hospital activities: a. Policy/procedure review and revision b. Helping Hand review and revision c. Patient/family advocacy12. Participates in clinic, hospital meetings, and shared governance/unit council.13. Participates & contributes to the professional development of peers and colleagues a. Completes peer reviews b. Serves as preceptor/resource c. Provides and/or coordinates continuing education programs d. Assists with clinical competency assessment.14. Coordinates research, quality improvement activities, and special projects.15. Participates in evidence based practice through application of policy, review of literature, and/or participation in nursing research.16. Provides assistance to and coverage for the Clinical Leader.17. Accepts and supports change in a positive manner.18. Plans and coordinates program activities to meet regulatory standards in one or more of the following: a. Environmental rounds b. OSHA activities c. HIPAA regulations19. Performs activities related to cluster/staff delegated initiatives a. Incident report follow-up b. Performance appraisal peer review c. Competency assessment20. Plans, implements and reviews program changes.21. Coordinates managed care activities specific to clinic.22. Practices professional nursing in accordance with ANA Code of Ethics for Nursing.
Qualifications
EDUCATION:Graduation from an ACEN or CCNE accredited school is required. LICENSURE: Licensed as a Registered Nurse in the State of Ohio.EXPERIENCEThree years pediatric clinical experience or equivalent combination of education/experience preferred.
