JOB SUMMARY

Recommend and assist access to medical, educational, social, vocational, housing, and more to support program goals. Initiate referrals to next level care, coordinate with physical health providers for whole-person care, and advocate in the best interest of clients. 

RESPONSIBILITIES

•Schedules and conducts intake assessments, including biopsychosocial assessment, level of care determination, safety planning, treatment planning, outcome measures, and relevant paperwork/data entry, to include in jails and inpatient settings.

•Develop and maintain referral, screening, intake & discharge tracking and reporting. 

•Attends County meetings as designated and appropriate to provide on-site facilitation of referrals.

•Conducts outreach as directed by the Program Manager.

•Knowledgeable of community resources and provides warm handoffs for referred clients not meeting eligibility.

•Acts as the liaison between the program and family/community agencies.

•Assists in Quality Assurance accuracy and Utilization Management.

•Submits documentation for timely billing and reimbursement. 

•Complete accurate and timely documentation, meet minimum billable requirements for direct client services if contractually required. 

•Facilitate placement including pre-placement visits, and transportation as needed.

•Shift coverage including part of the on-call team.

•Maintain confidentiality of client’s care.

•Other relevant duties as assigned. 

QUALIFICATIONS

•Master's degree or Bachelor’s degree with two years of experience in social services.

•Valid CA driver’s license and current auto insurance per contract requirements required. 

•Experience providing mental health and/or substance use disorder services preferred. 

•Cultural competency understanding required. 

•Bilingual preferred.

•Knowledge of Microsoft Office and EHR software preferred.
