Horsham Center for Jewish Life is searching for an experienced Social Worker to join the team on our Transitional Care Unit

Discharge planning experience with the geriatric population in a Skilled Nursing Facility is required for this position.Position: FT, Mon-Fri. 8am-4:30pmAbout Us

Be the change at The Horsham CenterJob Responsibilities
  • The TCU Social Worker works under the direction of the Director of Social Services.
  • Facilitates discharge planning from time of admission for 27 short-term rehab patients.
  • Completes inital assessments such as social history, care plan, trauma screening and MDS.
  • Leads care conferences with patient, family and interdisciplinary team.
  • Issues notices of Medicare non-coverage, arranges skilled homecare services, orders necessary medical equipment and arranges transportation as needed.
  • Knowledge of geriatric population.
  • Must be able to relate information concerning a patient's care plan/progess.
  • Coordinates with interdisciplinary team to address patient/family grievances.
  • Basic understanding of following step-by-step procedures.
  • Follows and understands written and oral directions.
Education & Qualifications
  • Must possess at a minimum a bachelor's degree from accredited school in Social Work or related field.
  • Experience with discharge planning in a Skilled Nursing Facility is required.
  • Must be knowledgeable of social services practices, procedures, laws, regulations and guidelines governing long-term care.
  • Ability to work independently or part of a group.
  • Strong communication skills.
  • Must have patience, a cheerful disposition and enthusiasm, as well as the willingness to handle challenging situations.
  • Effective problem solving skills.
  • Must possess the ability to deal tactfully with personnel, resident, family members, visitors, government agencies/personnel, and the general public.
